- Those who have participated in the Olympic Games, World Championships, or other international competitions, and who are employed by a public or private organization in Japan to receive a monthly remuneration of 25 yen or more for the purpose of promoting and improving the standard of amateur sports in Japan, are engaged in activities as an amateur sports player for that organization.

- 2. Applicant's resume and materials certifying his/her history (graduation certificate, documents certifying work history, etc.) as appropriate.
- 3. Materials showing competition history and results in the competitions as appropriate
- 4. Documents outlining the organization in Japan that will employ the applicant
- (1) 1 copy of registration certificate
- (2) One copy of balance sheet or income statement
- (3) Brochures or other materials outlining the company's overview, as appropriate

- 1. Document verifying the relationship between the applicant and the dependent (marriage certificate, birth certificate, etc.)
- 2. 1 copy of dependent's residence card or passport
- 3. Employment certificate of dependent 1 copy
- 4. Resident tax (or exemption) certificate and tax payment certificate (stating total income and tax payment status for the year) for dependents
*It will be issued by the ward office, city hall, or town hall in the city, ward, town, or village where you live as of January 1st.
*As long as the certificate shows both your annual gross income and tax payment status (whether or not you paid taxes), either one is acceptable.
*If you have just entered the country or have moved and your ward office, city hall or town hall has not issued a visa, please contact your nearest regional immigration bureau.
*If the dependent is already residing in Japan, please submit the dependent's employment certificate, dependent's residence tax taxation (exemption) certificate, and tax payment certificate if the dependent is already residing in Japan.

- 2. One copy each of the dependent's residence tax taxation (or tax exemption) certificate and tax payment certificate (one year's total income and tax payment status)
*Dependents' employment certificate, resident tax (exemption) certificate, and tax payment certificate are not required if the applicant and the dependent apply at the same time.
Points to note when preparing application documents
- 1. Please submit all certificates issued in Japan within the period of 3 months from issue date.
